Graduate Program Update Forms and Plans

These forms allow current students to make changes to their degree plans. Forms include readmission, reinstatement, and leave of absence, as well as changes to majors or catalog year.

ALL of these forms are fillable PDF forms and should be completed and submitted as such. Download the form, SAVE, complete the form and use electronic signature (student), SAVE and submit to gradcollegeforms@kennesaw.edu upon completion.

You will receive a response through your Ñý¼§Ö±²¥email within 2-3 weeks. 

  • If you were enrolled in graduate classes, but have not enrolled for at least three consecutive semesters, including Summer term, and wish to be readmitted submit this form.

    If you were accepted into a graduate program but did not attend please complete the through the Office of Graduate Admission.

    If your Ñý¼§Ö±²¥email is deactivated, we will contact you through your personal email. For readmission only.

    Readmit Deadlines:

    • Fall semesters - July 15th
    • Spring semesters - December 1st
    • Summer semesters - April 15th

  • A graduate student may be considered for reinstatement after sitting out the semester immediately following dismissal. The completed petition must be submittted to gradcollegeforms@kennesaw.edu.

    Spring semesters - December 1st
    Summer semesters - April 25th 

    You will be notified via your Ñý¼§Ö±²¥email when a decision has been made. 

  • For students experiencing unusual circumstances, a leave of absence can be requested. Note, an approved Leave of Absence is required for students enrolled in Thesis or Dissertation hours only. This form requires approval of the Graduate Program Coordinator and The Graduate College. Once your coordinator has approved email to gradcollegeforms@kennesaw.edu

  • Deadlines for Request for Program Changes /Concentration

    The request for a change of programs form is for currently enrolled students ONLY.

    • Fall 2025 semester - July 15, 2025 

    After July 15, 2025, the Graduate College no longer will accept program change forms. Students who wish to change their program of study must complete a new application with Graduate Admissions.

  • Current students who wish to change from their current catalog year. 

  • All requirements for a master’s degree program must be completed within six years. All requirements for a doctoral degree must be completed within ten years. The Graduate College may grant an extension of time for conditions beyond the student's control. Individual programs may have different requirements for that program's time limit.  Refer to program descriptions in this catalog for additional information.

  • Students may request transfer credit from a regionally accredited institution where graduate coursework was taken. Awarding of transfer credit is at the discretion of the program coordinator of the program and must be initiated by the program coordinator. Request for transfer credit MUST be submitted by the end of the student's second semester.

  • IMPORTANT: ALL requests for an add-on certificate must be submitted no later than four weeks prior to the beginning of a semester. Please complete the top portion of the form and email to gradcollegeforms@kennesaw.edu for processing. Requests received during student's final semester will not be processed.
